Microlife Medical Home Solutions, Inc. Launches the “Fast Track to the Patient-Centered Medical Home” Program Assisting Physicians with the Patient Protection & Affordable Care Act
Golden, CO---May 17, 2010 — Microlife Medical Home Solutions, Inc. (MiMHS) released the first edition of “Fast Track to the Patient-Centered Medical Home” at the American College of Physicians Scientific Meeting in Toronto, Canada. This new program is a turn-key approach for all physicians and providers seeking to become a National Commission for Quality Assurance (NCQA) Physician Practice Connections® --- Patient-Centered Medical Home (PPC-PCMH™) in 6 months or less. The Patient Protection & Affordable Care Act (H.R.3590) has appropriated funding for “medical home” pilots (Section 3021). The purpose of the bill is to evaluate a payment incentive and service delivery model that utilizes physician-directed primary care teams designed to reduce expenditures and improve health outcomes. Early data from the Patient-Centered Primary Care Collaborative (PCPCC) indicates medical homes can reduce health care expenditures by 15 to 20%, improve patient experiences surrounding care coordination and activation, increase staff morale, and improve primary care physician retention. A blended model of payment paid directly to each patient’s designated medical home would enable primary care physicians to redesign their offices to deliver high-quality preventive and chronic medical care with improved outcomes for all patients.
Dr. Hammond and his Westminster Medical Clinic in Westminster, Colorado achieved the highest NCQA PPC-PCMH Level 3 recognition as one of 16 pilots participating in the Colorado Multi-Stakeholder, Multi-State PCMH Pilot convened by the Colorado Clinical Guidelines Collaborative. Dr. Hammond added, “The changes we have made have benefitted everyone involved—the doctors, staff members from physician assistants to receptionists, specialists and most importantly, our patients. I feel the PCMH is the key to survival of Family Physicians.”
The “Fast Track to the Patient-Centered Medical Home“ program is currently available to all physicians and providers seeking NCQA PPC-PCMH recognition. The package includes a comprehensive binder & CD with all the reproducible templates needed to submit for NCQA PPC-PCMH Level 1 recognition.
